DOT PHYSICAL EXAMS: START HERE!

Our DOT Physical Exam procedure just got even easier! With the QR Codes above, all your paperwork can now be completed electronically, making your visit go faster and getting you your medical card faster, too! Simply open your phone's camera and hover it over the QR code, then click the link that appears and follow the prompts to completion. If you are having issues with the QR code, just let us know and we can send you a text message or email with the link instead. 

When you arrive at our clinic, check in at the desk and then have a seat and the doctor will call you in to the exam room to complete the physical.

We accept cash, check, or card payment for our services. Sometimes your employer may cover the fees of your exam, if this is the case, let us know as soon as possible and have their contact information readily available for us.
